At Adaptive Pathway, we believe every individual and family navigating disability or major life transitions deserves more than just support—they deserve a pathway forward that works.
We specialise in NDIS Support Coordination in the Southeast Melbourne, assisting participants and families to understand the system, connect with appropriate services, and take confident steps toward their goals. Our approach combines lived experience with clinical and professional expertise, delivering compassionate, practical, and effective support for individuals, families, schools, workplaces, and support providers.
Adaptive Pathway was founded by Greg and Diandra Charalambous, a husband-and-wife team dedicated to transforming how support is delivered in the disability and mental health sectors.
Based in Berwick, Victoria Greg is an NDIS participant, mental health coach, and psychology/neuroscience student. After losing his vision at age 9 due to a brain tumour, Greg rebuilt his life with resilience, strength, and community support. With over a decade of experience in personal training, mental health support, and advocacy, he brings firsthand insight into the gaps in support systems and how to bridge them. He now leads our Support Coordination services with a clear understanding of participants’ needs.
Diandra is a critical care registered nurse with extensive experience supporting individuals and families through complex medical and emotional challenges. Her background in rehabilitation, emergency, and critical care, combined with her skill in navigating family dynamics, brings structure, clarity, and compassion to every service we provide.
Together, Greg and Diandra have built Adaptive Pathway to offer more than just compliance—they provide real-world support that adapts to real lives.
